What to Bring

  • Proof of Canadian citizenship or immigration status (PR card, COPR, work/study permit)
  • Proof of Ontario residency — one document (original, printed or digital, from the accepted list)
  • Proof of identity with date of birth (e.g., passport, birth certificate)

How to Apply

  1. Visit a ServiceOntario centre in person — you cannot apply online.
  2. Bring all required documents (one proof of status, one proof of Ontario residency, and one proof of identity).
  3. Complete the OHIP registration form at the counter.
  4. A temporary card is issued immediately; the permanent green photo card arrives by mail within 4–6 weeks.
  5. Apply as soon as you arrive — there is no waiting period and no 153-day presence requirement before applying.

Tips

Keep your COPR or PR card as proof of status. Children born in Ontario to OHIP-eligible parents are covered immediately.
