What to Bring
- Proof of Canadian citizenship or immigration status (PR card, COPR, work/study permit)
- Proof of Ontario residency — one document (original, printed or digital, from the accepted list)
- Proof of identity with date of birth (e.g., passport, birth certificate)
How to Apply
- Visit a ServiceOntario centre in person — you cannot apply online.
- Bring all required documents (one proof of status, one proof of Ontario residency, and one proof of identity).
- Complete the OHIP registration form at the counter.
- A temporary card is issued immediately; the permanent green photo card arrives by mail within 4–6 weeks.
- Apply as soon as you arrive — there is no waiting period and no 153-day presence requirement before applying.

Keep your COPR or PR card as proof of status. Children born in Ontario to OHIP-eligible parents are covered immediately.
